Completed applications must be forwarded by mail or fax to the Mobility Plus office. It will take up to two weeks for the Mobility Plus office to assess your eligibility and let you know if you can use the service. All information will be kept confidential. If you are eligible, you will receive a registration card and number to use the service. If you require an attendant, this person must accompany you on all of your Mobility Plus rides. Attendants ride free-of-charge. All clients under the age of 18 must travel with an attendant, unless determined otherwise on a case-by-case basis. Attendants cannot be a Mobility Plus client.
You may qualify for Mobility Plus Family of Services (MP-1FS) status. This status means that depending on the locations you travel to, you may be booked using local YRT routes, Viva Rapid Transit, Community Bus routes and Mobility Plus door-to-door service for all or a portion of your scheduled trip.

To register with TTC Wheel-Trans, all Mobility Plus clients are required to have an interview assessment with Wheel-Trans. Mobility Plus will transport any client who resides in York Region to their Wheel-Trans interview at 5120 Yonge Street and 285 Cummer Avenue in Toronto. Please call a Trip Reservationist to book your ride.
If your application is denied, the Mobility Plus Eligibility Appeal Process offers you the opportunity to appeal the decision.
